Job Description:
- Assure compliance with state and federal licensing requirements, participate in the programs licensing process
- Participate in the screening process of potential clients. Complete all pre-admission and admission process
- Review client incident reports and take appropriate, timely corrective action. Notify Home Manager, Program Manager, and Director immediately of all critical incidents
- Develop and oversee implementation of Individual Service Plan (ISP) for each assigned client, including annual ISP review, team meetings and subsequent documentation distribution
- Complete required, daily, monthly, quarterly, and annual program documentation and other required reports in a timely manner
- Coordinate and assure service implementation with outside services through regular monitoring and progress reviews
- Coordinate and assure service implementation with Home Manager, Program Manager, or Director
- Facilitate client participation in community and recreational activities
- Coordinate with Physicians for the procurement and maintenance of adaptive equipment required by clients
- Assist in orientation and annual training of Direct Support Staff
- Work with the Home Manager and staff adherence to client schedule, use of specified material and accurate data collection methods
- Conduct regular audits of client data/progress notes and follow-up with staff on any errors or discrepancies
- Maintain open, frequent communication with Home Manager, Program Manager, and Director regarding concerns and general client status
- Adhere to the agency policies and procedures, work cooperatively with interagency departments respecting the established chain of command
- Assist in client evacuation in the event of a natural disaster
- Perform additional duties as assigned
- Bachelor's degree in field related to human services and 1 year experience supporting people with developmental disabilities
