Job Description:
- Responsible for assisting the with implementation of Patient’s Plans of Care
- Follows the direction of the Participation Coordinator with implementation of each patient’s Plan of Care and Treatment Program
- Supervises and monitors clients
- Monitors patient’s behavior and reinforces the Individual Behavior plan (IBP)
- Assists with all Functional Skill Building activities: Occupational Therapy Skill Building, Physical Fitness Skill Building, Pre-Vocational/Vocational Skill Building, Recreational Skill Building, and Animal Assisted Activity according to each client’s Plan of Care
- Assists the Life Skills instructor in implementing client’s program
- Monitor client activities of daily living and in instrumental activities of daily living
- Assist with organization of recreational and social programs of the facility.
- Completes all documentation and charting requirements
- Ensures guidelines are followed and interventions are carried out as specified
- Assists patients with maintaining an inventory list of all supplies
- Monitors clients checklist for self-medication
- Provides positive reinforcement during functional activities and projects
- Assists clients in maintaining living quarters in orderly fashion
- Upholds and ensures quality care
- Maintains effective communication with all members of the Interdisciplinary Treatment Team, clients, families, and visitors at all times
- Assist the patient, ensuring compliance with medical goal and objectives
- Monitors health and safety issues (i.e. .,vital signs, seizures, etc.)
- Monitors, observes, supervises and documents client’s self- administration of medications
- Preforms routine scheduled house cleaning as assigned
- Completes all annual Mandatory Educational requirements
- Preforms all duties assigned by management as necessary
- Responsible for driving company vehicles for the transportation of patients
- Requires a consistence presence at work
- Holds a diploma from an accredited secondary education school or GED
- Prefers one (1) year experience in a related field
- Prefers CNA, MA, and MHT
- Demonstrates sound judgment
- Maintains utmost degree of confidentiality regarding client information
- Possesses requisite level of technical knowledge to perform specified duties
- Requires knowledge of required regulatory agency standards related to health care organizations for brain injuries
