Job Description:
  • Specific job duties may involve maximum assistance, partial assistance, supervision and/or prompting of the client in the following areas according to their state Plan of Care and LHCL Care Plan(s)
  • Monitor Client's well-being
  • ADL's: Bathing, routine bodily functions and clean up, dressing/undressing, food preparation and storage, feeding/eating, transferring/lifting, personal hygiene/grooming, client specific household services, including laundry and linens
  • Health Care: Medications (administer, assist, prompt or supervise), skin care, nutrition (monitor and encourage proper food and fluid intake), scheduling medical appointments, transportation to and from medical appointments, medical needs/extension of therapy, attend to requests promptly
  • Community Supports: Transportation to client specific errands, grocery shopping, appointments, recreational outings, etc., social participation/communication/activities, money management, behavioral support and guidance, ambulation and exercise
  • Administrative Compliance: Proper documentation and completion of timesheets in a timely manner, attending to office requests promptly, following policy and procedures, communication with LHCL office
Qualifications:
  • Must be at least 18 years old.
  • Must be able to read, write and follow instructions in English.
  • Must satisfactorily pass all required and applicable State & Federal eligibility verification, including background, driving and other record checks.
  • Must be self-directing and able to work with little supervision.
  • Must be willing and eager to fulfill accepted case assignments.
  • Must be punctual and dependable, and professional at all times
  • Must maintain confidentiality
  • Must meet annual evaluation and education/training requirements
