Job Description:
  • Responsible for overseeing all aspects of assessing and meeting client needs in a manner that is consistent with MPCS' mission to help families, survive and live with cancer
  • Assess needs of clients, family members and caregivers in a conversational setting employing a personal approach and in a respectful, professional and non-clinical manner
  • Respond to the immediate needs of clients, family members and caregivers respectfully and compassionately
  • Document all communications with and services provided to clients, family members and caregivers in both client files and the MPCS client database
  • Promote and provide timely information on programs available to clients, family members and caregivers via written or verbal means
  • Assess effectiveness of services and programs offered and continually looks for ways to improve and expand service and program offerings
  • Provide coverage of client related events held on a monthly basis both during and after business hours (evenings and weekends). This includes, but is not limited to, Creative Expressions, Support Group Night, Spa Night, and SMILES for Miles
  • Attend special client and cancer related annual events such as, but not limited to, Camp Bluebird, Breast Cancer Retreat, National Cancer Survivors Day, Night at the BallPark and the Breast Health Symposium, as requested
  • Update Services Team Resource Binder as needed
  • Maintain awareness of community resources that could benefit clients, family members and caregivers undergoing cancer treatment, in recovery and beyond
  • Seek to stay informed of cancer related news which could impact services offerings as well as benefit clients, family members and caregivers
  • Maintain client confidentiality at all times
  • Remain sensitive to client, family members and caregivers' emotional needs as they deal with cancer diagnosis, treatment, and recovery issues
  • Communicate special and/or extraordinary client needs/requests to the Services Director as they arise; document process, resolution and outcomes
  • Perform all job responsibilities at the MPCS office, through the Outreach Program in local treatment facilities, and on Mobile Miles, a can which extends services to outlying communities throughout Acadiana (Actual assignments will be determined by the Services Director)
Qualifications:
  • The position requires experience in case management in a non-profit or social services organization
  • A Bachelor's or Master's Degree in social work is preferred
  • Candidate should be a good-natured self-starter, have strong interpersonal, communication, organizational and technology skills
